This short film focuses on the interactions between a doctor, social worker, and nurse who are trying to develop a comprehensive transitional care plan for a post-op patient who has developed an infection. Creating this plan is vital to their patient’s care because he is a senior who lives with dementia and has limited supports to ensure his optimal care. Unfortunately, due to role socialization, failure to actively listen and lack of critical reflexivity, conflict arises which makes it difficult for the team to collaborate.
