Description

The story involves an 85 year old female admitted to the hospital related to exacerbation of chronic obstructive pulmonary disease (COPD) secondary to a diagnosis of Pneumonia. The patient also has a diagnosis of dementia and diabetes. While in the hospital, the patient's dementia has worsened, making it unsafe for her to return to her home or stay alone once discharged from the hospital.
