Team 2: The Safety Plan
Description
Leah is a 4 year old girl who has developmental delays and requires some assistance at school. Leah is admitted to the hospital because she recently started to experience severe and sporadic seizures. With the help of her two nurses and doctor Leah was diagnosed with a seizure disorder, but with medication, she can return home. In saying this, for her to return to school the nurses are requesting direct support from her educational assistant and the early childhood educator in the classroom. This would include full attention and monitoring of Leah’s activities, following their transition/safety plan that they are implementing, and deeply familiarizing themselves with seizure protocol. There is a meeting scheduled to debrief on this. The two nurses, educational assistant, early childhood educator, and the school social worker will be attending this meeting. With the help of the school social worker dialogue is created amongst the interdisciplinary professions in order to de- escalate conflict in the situation, promote working collaboratively, and to successfully transition Leah back to school safely following her new diagnosis and the needs/responsibilities that come along with it.
