Food For Thought
Description
An interdisciplinary team talking about discharge planning for a patient in a medical unit. The patient, Mr. Ahoka, is an Indigenous 71 year old male who came in with diabetes and is now well enough to prepare for discharge planning. Problem arises when the topic of diet was addressed as there are multiple limitations in terms of patient food choices and accessibility. Priorities become conflicted between all levels of care: primary (socioeconomic approach), secondary (behavioural approach), and tertiary (physiological approach).
